A Fixed Smile When Dentures or Failing Teeth Are Holding You Back

Eating gets harder. Speaking feels less natural. Smiling can start to feel like something you control instead of something you enjoy.

All-on-X dental implants replace a full upper or lower arch with fixed teeth supported by implants, not removable plates or adhesives.

At Markham Family Dental in Harlan, Dr. Burkhalter plans full-arch implant treatment with 3D imaging, sedation options, and a clear treatment plan before anything begins.

what is All-on-X

All-on-X is a way to replace a full arch of teeth using four to six dental implants that hold a single, fixed bridge of new teeth. It rebuilds an entire upper or lower jaw at once, which makes it one of the most complete tooth replacement options available. The teeth are secured to the implants instead of resting on the gums like a removable denture, so you get a stable bite for speaking, smiling, and chewing. Your plan is built from a 3D scan of your bone, bite, and smile goals, and many patients leave the day of surgery with a fixed set of temporary teeth.
The X in All-on-X stands for the number of dental implants used to hold the arch, most often four, five, or six. All-on-4 uses four implants and All-on-6 uses six. The teeth are the same fixed bridge either way. Only the number of implants underneath changes, and it is chosen for your jaw and bite, not picked from a menu. More implants can add support on the upper jaw or for a stronger bite, while four well-placed implants carry a full arch reliably in many cases.

All Your Care in One Place: Our In-House Lab

Full-arch implants usually mean a chain of offices: one place for the consultation, an oral surgeon for the surgery, a separate provider for sedation, and an outside lab for the teeth. At Markham, all of it happens in one place with one team. Dr. Burkhalter plans the case, places the implants, provides IV sedation, and oversees your teeth, which are designed and crafted right here in our own in-house dental lab. That is rare in western Iowa, and it means nothing is handed off, no referral is needed, and your final teeth are not waiting in a shipping queue to a distant lab.
Everything in one place:
Your free consultation and in-house CBCT 3D scan
The implant surgery, with no referral to an outside oral surgeon
IV sedation, provided here for your comfort
Your temporary and final teeth, crafted in our in-house lab
Every follow-up, adjustment, and healing check

The Dental Implant Process

1

Free Consultation & 3D Imaging

Your journey starts with a no-pressure consultation with Dr. Burkhalter. Using our in-house CBCT 3D imaging technology, we capture a precise picture of your bone structure, jaw anatomy, and oral health. This lets us plan your implant placement accurately before anything begins, and identify whether any preparatory steps like bone grafting are needed.
This visit is completely free. You will leave with a clear picture of whether All-on-X is right for you and what your personalized treatment looks like.
2

Implant Placement and Same-Day Temporaries

Dr. Burkhalter places four to six titanium implants directly into your jawbone. IV sedation and local anesthesia are both available — we will not begin until you are fully comfortable.
Once the implants are in, temporary fixed teeth are attached the same day. You walk out of our office with a full smile. Not a gap. Not a plate. Teeth you can use while your implants heal over the following months.
3

Healing and Osseointegration

Over the next three to six months, your implants fuse with the bone in a process called osseointegration. This is what makes the entire structure permanent. You will return for regular check-ins so Dr. Burkhalter can monitor your progress. During this phase, a soft diet is recommended.
Most patients manage this period with far less difficulty than they expect. The temporary teeth look good and function well enough for everyday life.
4

Final Permanent Restoration

Once your implants have fully integrated with the bone, your permanent bridge is fitted. It is custom-crafted for your face, your bite, and your natural appearance. We fine-tune the fit and verify your bite before you leave. This is the restoration you will have for decades.

What Are All-on-X Teeth Made Of?

Built to Last
Zirconia resists chips, stains, and the force of a full bite, so it holds up to everyday eating for years.
Zirconia Teeth
Your visible teeth are crafted from solid zirconia, one of the strongest and most natural looking materials in dentistry.
Titanium Implants
The teeth anchor to titanium implants that fuse with your jawbone and work like natural tooth roots.
Made in Our Lab
Your bridge is shaped, shaded, and fine-tuned in our own in-house lab to match your face and smile.

Recovery and Aftercare

Most patients are back to everyday life within a few days, with some soreness and swelling that settle over the first week. Because you leave with fixed temporary teeth, you start on softer foods and ease back to a normal diet as the implants heal over three to six months. We see you for follow-ups during this time, and our team is a phone call away if anything comes up. Simple daily cleaning around the bridge keeps everything healthy.
