Markham Family Dental Associates, Inc., is a premier dental practice located at 2105 Pine Street in Harlan. Established in 1990 by Dr. Phillip
Markham, our practice has been providing the finest quality dental care to the Shelby County area for more than 30 years. Our newly remodeled, handicap-accessible office features the latest in imaging technology, in-office crown fabrication, and our new surgical suite.

We are outfitted with CBCT 3D imaging and 2D radiography as well as Cad Cam and other advanced digital technologies. These technologies
allow us to more accurately diagnose and treatment plan for each individual patient. With the help of our digital technologies, we scan,
design, and fabricate ceramic crowns, in-lays, and on-lays in our office, enabling more rapid and more affordable treatment for broken or
severely decayed teeth.

Amongst the newest updates to our office is our fully equipped surgical suite, outfitted to handle advanced procedures and routine extractions, wisdom teeth extractions, to implant-retained prosthetics and sedation dentistry. This allows us to perform many of the most advanced surgical procedures in our office in Harlan for less cost than you would find in the city.

Appointments

The first step towards a beautiful smile is to schedule an appointment. We have made requesting an appointment a simple process by either contacting our office by phone or complete the appointment request form. Our scheduling coordinator will contact you to confirm your appointment If, for any reason you cannot keep a schedule appointment, or will be delayed, please call us as soon as possible.

Insurance and Billing

At Markham Family Dental Assoc., we strive to provide the best dental care at an affordable, competitive price. We accept most dental insurance plans, however we do not participate as a PPO (Preferred Provider Organization) or HMO (Health Maintenance Organization). We will file a claim with your dental insurance company and track the claims on your behalf. Please make sure to keep us informed of any changes to your insurance plan and bring an up-to-date copy of your card(s) with you to each appointment. You are ultimately responsible for the fees charged by our office, no matter what your insurance coverage may be. If you have secondary insurance, the remaining copay will be subsequently filed after primary payment has been received. When all insurance has been paid and a balance remains you will receive a statement for the difference.

Treatment plans can be overwhelming at times, so we do send in prior authorizations to your insurance company when needed. This ensures that you will see what out of pocket portion you may be liable for. Ultimately you make the final decision.

We recommend that you check with your insurance provider when renewing your insurance. Important questions to ask are… what is the maximum allowed benefit per year, deductible, is there a waiting period before benefits begin and what procedures are covered by the insurance policy. The most that a dental office can do with your insurance is file a claim. Ultimately, the responsibility for coverage and payment remains with the patient.


MEDICAID OR STATE INSURANCE
At present time we are not accepting any new adults or children with Medicaid. If it has been longer than 3 years since you have been seen at our office, you would be considered a new patient. 

Payment Options

For your convenience we accept Visa, Mastercard, Discover Card & American Express. We also accept debit cards, HSA and FLEX cards, cash and local checks. We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered. If you feel you will be unable to pay for a service or procedure, speak with our front office staff about available financing options. This must be done prior to the procedure.

Please remember that you are fully responsible for all fees charged by this office regardless of your insurance coverage.

We will send you a monthly statement. If you have dental insurance, we are happy to file the claim with your insurance company. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect payments made by your insurance company during that time period. Any remaining balance after your insurance has paid its portion is your responsibility. Your prompt remittance is appreciated. Arrangements can be made for a monthly payment plan, but that must be implemented prior to any actual procedures.

We are now also offering online billing and bill pay. Statements can be sent directly to your e-mail. From there, you will be connected to a secure payment portal where you can make a payment or pay your balance in full. If you are interested in our online billing or online bill-pay, please talk to a front office staff member to ensure that we have a current e-mail address for you. 

New Patient Info

We are always welcoming new patients at our office! If you would like to schedule an appointment with us please either call our office or use the request appointment online. If you use the online appointment request form, please indicate on the form that you are a new patient. (Note: the online form is for REQUESTED appointments, a member of our staff will call or text you to confirm the time and date of your appointment.)

Arrival Time
For your first appointment, please try to arrive at least 10 – 15 minutes prior to your scheduled appointment time. We will have a short health history form, that we will need you to complete, if this was not initially done online. This is to ensure that we can provide you with the highest quality of dental care possible.


Pre Med
For certain health conditions, you may require a medication prior to your appointment. If this is the case, please contact us at least two business days prior to your appointment so that we can arrange the necessary prescriptions.
